Zusammenfassung

Früherkennung, Diagnostik und Therapie maligner Erkrankungen erfahren durch die Weiterentwicklung und Kombination bekannter und neuer Strategien eine stetige Verbesserung. Trotzdem zeigen viele Patienten mit malignen Tumoren bereits bei Diagnosestellung ein fortgeschrittenes Tumorstadium mit lokaler Infiltration und/oder Fernmetastasierung, das einem kurativen Therapieansatz entgegensteht. Es stehen aber zahlreiche interdisziplinäre palliative Behandlungskonzepte zur Verfügung. Die Chirurgie nimmt hier ebenso wie in der kurativen Tumortherapie eine zentrale Rolle ein. Der vorliegende Beitrag stellt die Möglichkeiten und Grenzen der palliativen Viszeralchirurgie und ihre Integration in multimodale Therapiekonzepte bei diversen Tumorentitäten dar. Darüber hinaus wird die Therapie der Peritonealkarzinose und die Indikation zur Peritonektomie in Kombination mit intraperitonealer und systemischer Chemotherapie ausführlich erörtert.

Abstract

Despite constant improvements in diagnostics and cancer therapy, numerous patients with malignant tumors present with advanced tumor stages with local infiltration and/or distant metastasis at the time of initial diagnosis. Many of these patients only qualify for palliative therapy. Surgery plays a pivotal role in different interdisciplinary palliative treatment strategies. In the present review, the surgical therapy options for the treatment of different tumor entities are discussed. Moreover, the therapy of peritoneal carcinomatosis including peritonectomy in combination with intraperitoneal and systemic chemotherapy is reviewed.
